Many couples believe pregnancy will happen naturally within a few months, so it can be frustrating and emotionally exhausting when it doesn't. As a Gynaecologist and Infertility Specialist, I often meet couples who wonder whether they should continue waiting or seek medical advice. The good news is that infertility is often treatable once the underlying cause is identified. Seeking help early does not automatically mean you need IVF. In many cases, a careful evaluation and appropriate treatment can significantly improve your chances of conceiving naturally or with simpler fertility treatments than couples expect.
I want couples in Thrissur to understand that coming for a fertility evaluation is not an admission that something is seriously wrong. It is simply the most logical next step when pregnancy has not happened within the expected timeframe. It allows us to understand the picture on both sides, identify anything that needs attention, and put a plan in place that is appropriate for your specific situation.
When Is the Right Time to Consult a Specialist?
Generally, couples should consider an infertility evaluation after one year of regular unprotected intercourse without pregnancy. Women above 35 years of age, those with irregular menstrual cycles, previous miscarriages, endometriosis, or known reproductive conditions should seek medical advice earlier.
If there is a known condition on either side, such as a previous diagnosis of PCOS, a history of pelvic inflammatory disease, a previous surgery involving the reproductive organs, or a semen analysis that has flagged concerns, then earlier evaluation is appropriate regardless of how long you have been trying. The timing guidance exists for couples with no known concerns. For those who already have a relevant history, there is no benefit in waiting.
Fertility Is a Couple's Journey
Infertility does not affect only women. Male factors contribute to a significant number of infertility cases, which is why I always encourage both partners to undergo evaluation together. This helps identify the actual cause and allows us to choose the most appropriate treatment.
In my practice, I frequently see couples where the focus has been entirely on the female partner for months, sometimes years, before a semen analysis is done and a significant male factor is identified. A basic semen analysis is a simple, non-invasive investigation that should be part of every infertility evaluation from the outset. Starting with a complete picture of both partners saves time, reduces the emotional load of repeated investigations, and gets us to the right treatment faster.
Not Every Couple Needs IVF
One of the biggest misconceptions I hear is that visiting a fertility specialist means IVF is inevitable. That simply isn't true. Depending on the diagnosis, treatment may include lifestyle modifications, ovulation induction, medication, IUI, or assisted reproductive techniques when appropriate.
For many couples, particularly those where the primary issue is ovulation irregularity or mild male factor, ovulation induction combined with timed intercourse or IUI produces very good outcomes without the cost, complexity, or physical demands of IVF. I always begin with the simplest appropriate treatment and escalate only when there is a clear clinical reason to do so.
