My Long-Term Ventilation Course at Great Ormond Street Hospital, UK, trained me specifically in the clinical management of children who require home ventilation for neuromuscular disease, central hypoventilation, and severe chronic lung disease. Cough is the presenting symptom in most of the children who come to me for the first time. For many of them, it has been present for weeks or months and has been managed with repeated antibiotic courses that produce temporary improvement and then recurrence. A Paediatric Pulmonologist's approach to cough is systematic and diagnostic rather than presumptive - I want to know whether the cough is from the nose dripping into the throat, from the airways in asthma, from the lung parenchyma in pneumonia or bronchiectasis, or from the airway itself in conditions like tracheobronchomalacia. Flexible bronchoscopy, where I pass a thin camera through the nose or mouth into the airway under sedation, allows me to see what no imaging study can show.

These are conditions where the right management decision at diagnosis changes the trajectory of lung function across decades of a child's life. Sleep medicine is the third pillar, and it is one that is profoundly underserved in paediatric practice: a child with untreated obstructive sleep apnoea is not just a child who snores - they are a child whose sleep is fragmented, whose brain is intermittently deprived of oxygen during the night, and whose daytime functioning, behaviour, learning, and growth are all impaired as a consequence.
