Laser treatment and topical oestrogen therapy work by different mechanisms, and for many women in my practice in Beed, the best outcome comes from using both together rather than choosing one over the other. Topical oestrogen therapy, applied directly to the vaginal tissue as a cream, ring, or tablet, works by replacing the oestrogen that has been lost at menopause. It restores the vaginal tissue's responsiveness to oestrogen, increasing mucosal thickness, lubrication, and the restoration of normal vaginal pH. It is an evidence-based, effective treatment and is safe for the vast majority of women, including most women with a history of breast cancer when used at the low local doses prescribed for vaginal atrophy. IntimaLase GSMCare laser treatment works differently. Rather than replacing the missing hormone, it delivers a controlled photothermal stimulus to the vaginal mucosa that activates the tissue's own collagen remodelling and regenerative response. The tissue rebuilds itself through its own biological repair mechanisms. Laser treatment is particularly valuable for women who cannot or prefer not to use hormonal therapy, and it can be combined with topical oestrogen in women who want a more comprehensive approach. In my practice at Shri Kripalu Hospital, Beed, I assess each patient's clinical history, symptoms, hormonal status, and any contraindications before recommending whether laser alone, topical oestrogen alone, or a combination approach is the most appropriate management plan.
