Failed IVF - Not the End of the Road

A failed IVF cycle is one of the most difficult experiences a couple can go through, and one of the most common conversations I have at Oasis Fertility, Hyderabad. It does not mean the journey is over. In most cases, understanding what went wrong and adjusting the plan makes a meaningful difference to the next cycle. 

Discuss with your doctor to understand the cause of failed IVF and the further options available. 

Possible reasons for failed IVF could be poor quality of the embryo, failed hatching of the embryo, non-receptive endometrium, clotting defects, or immune factors. The above factors can be tested depending on the patient's history and appropriate measures can be taken in the following cycles for better chances. 

What can be done to improve chances in the next cycle? 

  • Change of drugs and protocol: Sometimes changing the drugs and the stimulation protocol yields better quality eggs and embryos.

  • Immunotherapy: Treatments like intralipids and immunoglobulins can be given.
  • Endometrial Receptivity Assay (ERA): This test is done to know if the endometrium is receptive or not and also to know the window of implantation for the planned embryo transfer.
  • Assisted Hatching of Embryo: Laser assisted hatching is a method where the thick shell around the embryo is chipped and made thin at a few points so that the embryo can easily hatch out and implant.
  • Embryo Glue: It is a special culture medium used during embryo transfer for better implantation and better success rates. It is considered in women with repeated failed IVFs.
  • Donor: In case of poor embryo quality due to poor egg quality or very poor sperm, opting for donor egg or sperm has better success rates. 

Lifestyle modifications: Simple changes in lifestyle help in achieving better results. 

  • Regular exercise
  • Taking a balanced diet rich in antioxidants
  • Maintaining optimal BMI
  • Adequate sleep
  • Stress management 

Diagnostic Hysteroscopy: Hysteroscopy before the embryo transfer helps in detecting any unidentified micro polyps, septae, and adhesions resulting in a failed IVF cycle.
