The allergic march describes the sequential development of allergic conditions in a genetically predisposed individual  -  typically eczema in infancy or early childhood, followed by the development of IgE-mediated sensitisation to food and then airborne allergens, then allergic rhinitis, and in a proportion of those with rhinitis, the progression to asthma. These are not separate conditions that happen to occur in the same person  -  they are sequential expressions of the same underlying allergic immune dysregulation, and the allergen sensitisation that drives eczema often persists and manifests in a different anatomical site  -  the nasal mucosa  -  once the child is exposed to airborne allergens in the amounts that produce nasal disease. If you had eczema as a child and now have allergic rhinitis as an adult, the sensitisation profile  -  the specific allergens driving your immune response  -  is worth evaluating even if you believe you have grown out of the eczema. In Chennai's environment, where house dust mite sensitisation is ubiquitous and Prosopis juliflora pollen adds a seasonally significant load, the allergen that sensitised you as a child may well be the same one driving your adult rhinitis. I am planning a skin aesthetic treatment but I have sensitive skin and a history of allergies - what should I know?

Having a history of skin allergy, atopic dermatitis, or contact allergy does not mean you cannot have aesthetic treatments  -  it means your skin assessment before treatment needs to be more thorough than for patients without this history, and the selection of products and procedures used in your treatment needs to account for your known sensitivities. Patients with atopic dermatitis have a compromised skin barrier that increases the risk of irritant and allergic reactions to topical products applied during and after procedures. Patients with contact allergy may be sensitised to specific preservatives, fragrances, or ingredient categories that appear in some topical anaesthetic preparations, peeling agents, or post-procedure skincare. Patients with a history of urticaria or angioedema need specific discussion about the potential for reactions to injectable agents. As both an allergy fellowship-trained clinician and an aesthetic physician, I assess skin and allergy history before any procedure, identify any relevant sensitivities through patch testing or clinical history review, select the appropriate formulations and procedures for each patient's skin profile, and discuss the expected recovery in the context of the patient's skin reactivity. This is not a reason to avoid aesthetic treatment if you want it  -  it is the reason to have it done by a clinician who understands the intersection between your allergic history and the safety of your aesthetic care.

My eczema keeps flaring up despite using the steroid cream my doctor prescribed - what else can be done?

Topical corticosteroid creams treat the inflammation of an eczema flare effectively  -  reducing redness, itch, and skin inflammation during the acute phase  -  but they do not address the allergen or trigger that caused the flare to occur. When eczema recurs repeatedly after the cream has cleared the flare, the question is what is causing the flares, not whether the cream is working. The most important addition to the management of recurrent eczema flares is trigger identification. The common triggers that allergy evaluation reveals are: food allergy, particularly to egg, milk, wheat, and tree nuts in children, and shellfish, tree nuts, and wheat in adults, where the food-eczema connection is often not recognised because the flare does not occur immediately after eating; environmental allergen contact, where house dust mite and cockroach sensitisation in Chennai's environment produces persistent skin inflammation that topical treatment controls only temporarily; and contact allergy to specific ingredients in skin care products, including preservatives, fragrances, and emulsifiers in topical preparations  -  including, sometimes, ingredients in the very products being used to manage the eczema. Patch testing identifies contact allergens. Skin prick testing and specific IgE identifies food and environmental allergens. The management plan after trigger identification is specific: remove the identified trigger from the diet or environment, optimise emollient use to restore the skin barrier that eczema disrupts, use topical anti-inflammatory treatment as needed, and treat any identified environmental sensitisation with appropriate avoidance measures. Identifying the trigger is the step that turns recurrent eczema into a controllable condition.

My nasal congestion is worst in the mornings - is this allergy or something structural in my nose?

Morning nasal congestion that is at its worst on waking and gradually improves over the first hour or two of the day is one of the most consistent clinical patterns of house dust mite allergic rhinitis  -  and house dust mite is the dominant perennial allergen in Chennai's humid coastal climate. The explanation is straightforward: house dust mite concentrations are highest in the bedroom environment  -  the mattress, pillows, and bedding where mites live and reproduce  -  and the overnight period of sleep is the longest sustained allergen exposure a sensitised patient experiences. Waking congestion reflects the accumulated nasal mucosal inflammation from those hours of exposure, and the improvement through the day reflects movement to lower-allergen environments and the natural diurnal variation in nasal airway congestion. If your morning congestion follows this pattern and is accompanied by sneezing, nasal itch, and a clear runny nose, allergic rhinitis from house dust mite sensitisation is the most likely diagnosis and allergy testing will confirm it. If the congestion is more constant throughout the day, present without sneezing or itch, and associated with difficulty breathing through the nose on a persistent basis, a structural component  -  deviated nasal septum, inferior turbinate hypertrophy, or nasal polyps  -  may be contributing alongside or instead of allergy. As both an ENT surgeon and an allergy fellowship-trained clinician, I assess both dimensions at the same consultation: nasal endoscopy evaluates the anatomy while allergy testing evaluates the immune sensitisation, giving a complete picture of what is driving each patient's nasal obstruction.
