There is a recurring pattern I see in my practice in Margao. A patient who has been having ear infections every few months. A patient whose nose has been blocked for years and who has learned to consider it normal. A patient whose voice has changed and who cannot pinpoint why. Each of them has been to various clinics and been given treatment for the presenting episode - antibiotics for the ear infection, decongestants for the nose, a referral to a speech therapist for the voice - but no one has asked the connecting question: why do these symptoms keep coming back? Goa's environment is beautiful, but it is also allergenic. The high humidity that characterises the coastal and post-monsoon climate supports year-round house dust mite proliferation at levels higher than drier inland regions. Mould is pervasive in older Goan homes, which retain moisture in walls and roofing. Cockroach allergen in urban Margao households is significant. And the tourist population brings additional patterns - visitors who develop symptoms during their stay and cannot understand why their nose or ears are behaving differently in Goa.

When I identify and treat the allergy, I change the trajectory of the ENT disease - fewer ear infections, better sinus control, and a voice that improves because the post-nasal drip driving it has been addressed.
