My voice has been hoarse for several weeks - should I see an ENT specialist?

Yes  -  hoarseness or a voice change that has been present for three weeks or more should be evaluated by an ENT specialist. The three-week threshold exists because the most common causes of short-term hoarseness  -  viral laryngitis from a cold, voice strain from shouting or excessive use  -  typically resolve within two to three weeks. Persistent hoarseness beyond this window indicates something else is driving the change, and the range of causes includes both benign and significant conditions that cannot be distinguished without clinical examination. In my ENT practice in Goa, the most common causes of persistent voice change I identify are: laryngopharyngeal reflux, where stomach acid reaches the throat and larynx and produces inflammation, chronic throat clearing, and a raspy voice  -  this is very common and responds well to dietary and medical management; post-nasal drip from allergic rhinitis, where constant dripping of mucus onto the vocal cords produces voice fatigue and hoarseness that clears briefly after throat clearing then returns  -  this is the allergy-voice connection that my AASC training helps me identify and address; and vocal cord nodules or polyps from voice strain or overuse. Less commonly, hoarseness may indicate a laryngeal lesion that requires examination of the vocal cords with a flexible scope to evaluate. Any voice change lasting more than three weeks deserves a proper ENT assessment  -  it is almost always benign, but it needs to be seen.

What is Meniere's disease and why does Goa's climate seem to make my symptoms worse?

Meniere's disease is a disorder of the inner ear caused by excess endolymphatic fluid  -  a condition called endolymphatic hydrops  -  that produces episodic attacks characterised by the same three features: rotational vertigo that is genuinely disabling during the attack and lasts from 20 minutes to several hours, tinnitus (a ringing or roaring sound, typically low-pitched, in the affected ear), and a fluctuating low-frequency hearing loss that may worsen with each attack over time. Between episodes, patients may feel entirely well, which makes the unpredictability of attacks particularly distressing. The observation that Goa's coastal climate seems to worsen symptoms is consistent with what many Meniere's patients report worldwide  -  barometric pressure changes and high ambient humidity can influence the pressure dynamics of the inner ear fluid compartment, triggering or intensifying attacks in susceptible individuals. During the monsoon season in Goa, when atmospheric pressure changes are frequent, some patients notice a clustering of episodes. Management is focused on reducing the fluid pressure: dietary salt restriction, avoiding caffeine and alcohol, diuretic therapy, and betahistine are the established first-line approaches. Vestibular suppressants manage the acute attack. For patients whose attacks continue despite medical management, more advanced interventions including intratympanic injections can be considered. The goals of management are reducing attack frequency, protecting residual hearing, and improving quality of life.

My ear keeps getting infected after swimming - how do I prevent swimmer's ear?

Swimmer's ear  -  otitis externa  -  is the outer ear canal infection produced when water trapped after swimming creates a warm, moist environment in which bacteria, most commonly Pseudomonas, thrive. In Goa, where sea and pool swimming are part of daily life across a large proportion of the population, otitis externa is one of the most frequent ENT presentations I see at my Margao center. Symptoms are distinctive: ear pain, particularly on pulling the outer ear or pressing the tragus, ear discharge, and a sense of blockage or muffled hearing  -  without the fever that typically accompanies middle ear infection. The most important prevention measures are keeping the ear canal dry after swimming, tilting the head to each side after water exposure to allow water to drain, and avoiding cotton buds  -  which push debris deeper into the canal, damage the protective skin lining, and increase infection risk significantly. Patients who swim frequently benefit from prophylactic acidifying ear drops  -  a dilute acetic acid solution  -  applied after swimming to maintain the mildly acidic ear canal environment that inhibits bacterial growth. When otitis externa is already established, treatment with topical antibiotic and steroid ear drops resolves the infection in most cases, with oral antibiotics reserved for severe infections or patients with risk factors. Why do I keep getting sinus infections in Goa even though I take antibiotics every time?

In Goa's high-humidity environment, recurrent sinusitis that responds to antibiotics and returns within weeks is one of the most common patterns I see at my center in Margao  -  and in the large majority of these cases, the antibiotics are treating the secondary bacterial infection without addressing the primary allergic inflammation that keeps creating the conditions for that infection to occur. The mechanism: allergic rhinitis from house dust mite, mould, or cockroach sensitisation produces mucosal oedema in the nasal passages that chronically obstructs the sinus drainage passages. Bacteria grow in the stagnant mucus behind the obstruction. Antibiotics clear the bacteria. But the allergic inflammation is still there, the obstruction returns, and the cycle repeats. The solution is identifying the specific allergen driving the nasal inflammation through clinical assessment and allergy evaluation, and addressing it with appropriate treatment  -  allergen avoidance in the Goan home environment, intranasal anti-inflammatory therapy, and where indicated, allergen immunotherapy to modify the underlying immune response. When the allergy is treated, the sinusitis episodes reduce in frequency and severity, often dramatically.
