Yes  -  allergen immunotherapy is a well-established treatment option for patients with chronic sinusitis that has a confirmed allergic component, and its role is particularly important in patients who have had sinus surgery and want to reduce the risk of recurrence. The mechanism is direct: allergen immunotherapy gradually reduces the IgE-mediated immune response to the specific allergen, producing a shift from allergic Th2-type reactivity toward immune tolerance. Over the course of the treatment  -  typically three to five years  -  this reduces the allergen-driven mucosal inflammation that feeds sinusitis episodes and nasal polyp formation. The evidence for allergen immunotherapy in allergic sinusitis is strongest for house dust mite sensitisation, which is the most prevalent sensitisation in Bengaluru ENT patients, and for pollen sensitisation in patients with seasonal exacerbations. For patients who have had FESS and want to protect their surgical outcome, immunotherapy is one of the most clinically evidence-based interventions available  -  reducing the inflammatory driving force means extending the period of good sinus function after surgery. It is offered as subcutaneous injections in clinic during a build-up phase followed by monthly maintenance, or as daily sublingual drops or tablets at home. FAAI training informs my selection of appropriate candidates  -  not every allergic rhinitis patient needs immunotherapy, but those with significant allergen-driven sinusitis who are committed to a multi-year treatment course achieve outcomes that pharmacotherapy alone cannot sustain.

Two weeks is already the point at which an ENT evaluation is appropriate. The general guidance is that a neck lump persisting beyond two to three weeks without an obvious cause  -  a current upper respiratory infection, for example, which typically produces tender, symmetrical lymph node swelling that resolves with the infection  -  should be clinically assessed by an Otolaryngologist. The reason is not to cause alarm, but to conduct the systematic clinical evaluation that correctly identifies the nature of the lump and guides the next step. The large majority of neck lumps in adults and children are benign: reactive lymphadenopathy from a recent or current infection, a benign cervical lymph node, a thyroid cyst or nodule, a salivary gland retention cyst, or a congenital lesion such as a branchial cyst or thyroglossal duct cyst. The clinical assessment that distinguishes these from more significant pathology includes the lump's location in the neck (anterior, posterior, lateral, submandibular), its consistency (soft, rubbery, or firm and fixed to underlying structures), its size, whether it is growing, and whether it is associated with symptoms such as persistent sore throat, hoarseness, difficulty swallowing, unexplained weight loss, or night sweats. When the clinical features suggest investigation beyond examination alone, neck ultrasound and fine needle aspiration cytology are the standard next investigations. Seeking ENT assessment of a neck lump is not an overreaction  -  it is the appropriate step that ensures nothing significant is missed while most lumps are correctly reassured as benign.

FESS effectively removes nasal polyps, restores sinus drainage, and produces significant improvement in breathing, smell, and sinus symptom burden  -  for most patients this improvement is substantial and the surgery is clearly worthwhile. However, 'permanent cure' is not the correct framing for nasal polyposis, and understanding why matters for long-term management. Nasal polyps are not tumours to be excised and forgotten  -  they are the end-product of persistent mucosal inflammation, driven by eosinophilic inflammatory processes that are frequently but not always allergic in origin. Removing the polyps does not remove the inflammatory mechanism that produced them. When that mechanism is active and untreated, new polyps form in the open sinus spaces that surgery created, often within months to a few years. The factors that most strongly determine how long FESS benefit lasts are: consistent post-operative intranasal corticosteroid spray use  -  which reduces mucosal inflammation and slows polyp reformation  -  and formal allergy evaluation to identify and treat any allergen sensitisation contributing to the inflammation. Patients who have FESS and then stop their nasal spray, or who have never had their allergy status evaluated, tend to develop polyp recurrence significantly sooner than those who follow a structured post-operative management programme. The surgery itself is one part of a longer treatment relationship, not a single intervention that produces indefinite benefit without ongoing care.

The Fellowship in Allergy, Asthma and Immunology  -  FAAI  -  is a fellowship-level postgraduate qualification in allergy and clinical immunology that provides a broader and deeper training framework than a specialist course in allergy and asthma. While standard allergy courses provide the clinical framework for common allergy evaluation and management, an allergy fellowship encompasses the full scope of clinical immunology  -  including the evaluation of primary immunodeficiency conditions, the mechanisms of immune dysregulation that drive autoimmune and inflammatory disease, and the more complex end of allergy diagnosis including component-resolved diagnostics and the clinical decision-making that guides allergen immunotherapy in complex cases. For an ENT specialist, holding FAAI rather than only a course-level allergy credential means that when a patient presents with recurrent sinusitis and persistent post-nasal drip and their allergy test comes back showing sensitisation to multiple allergens, the interpretation of that result  -  which sensitisations are clinically relevant, which require management, and whether immunotherapy is appropriate  -  is informed by a fellowship-level understanding of immune mechanisms, not only a course-level familiarity with testing. In practical terms, it means more precise allergy diagnosis and more individualised treatment planning.
