Does my child really need a spacer device with their inhaler, or can they just use it directly?

A spacer device significantly improves how much medication actually reaches a child's airway compared to using an inhaler alone, which is why I recommend one for every child regardless of age. Without a spacer, much of the medication can be lost due to poor coordination between pressing the inhaler and breathing in, a timing issue that is especially common in young children and during a stressful flare-up when breathing is already difficult. With a spacer, the medication is held in a chamber and can be inhaled over several breaths, removing the need for that precise timing and ensuring a much higher proportion of the dose reaches the lungs rather than the back of the throat. I check spacer technique at every visit, since even small errors in how it is used, such as not sealing the mouthpiece properly or breathing too quickly, can meaningfully reduce how effective the medication is during an actual attack.
