Am I a Candidate for Bariatric Surgery? How to Know if Weight Loss Surgery Is Right for You

The most common question I hear from patients considering weight loss surgery is not about the procedure itself. It is this: am I a candidate? The answer depends on a structured clinical assessment, not a simple number on the scales. As a bariatric surgeon at NiSH Clinic, Jayanagar, Bengaluru, with over 8,000 surgeries performed, I see patients every day who meet the criteria for bariatric surgery eligibility but have been told to simply try harder with diet and exercise. I want to be clear about what the evidence actually says.

The clinical criteria for bariatric surgery

International guidelines recommend bariatric surgery for patients with a BMI of 40 or above, or a BMI of 35 or above when accompanied by obesity-related health conditions such as type 2 diabetes, high blood pressure, obstructive sleep apnoea, joint disease, or fatty liver. In the Indian context, these thresholds are often lower — BMI 32.5 with co-morbidities is recognised as a reasonable threshold given that Indian body composition carries greater metabolic risk at lower BMI levels than Western populations. Patients must also have made genuine attempts at non-surgical weight management, be medically suitable for surgery, and be committed to the lifelong dietary and nutritional changes that weight loss surgery requires.

What conditions does bariatric surgery actually help?

The benefits of bariatric surgery extend well beyond weight reduction. Type 2 diabetes is the condition most dramatically affected: in many patients with obesity and diabetes, blood sugar levels normalise within days of gastric bypass surgery, before significant weight loss has occurred. This is a hormonal effect, not simply a caloric one, and it is one of the most compelling reasons to consider surgery early rather than late. High blood pressure frequently improves or resolves. Sleep apnoea often remits completely. Joint pain reduces as load-bearing joints are relieved. The combination of these benefits means that bariatric surgery Bengaluru is not simply a cosmetic or lifestyle procedure — it is a metabolic intervention with a strong evidence base for improving long-term health outcomes.

Who is not suitable? Patients with uncontrolled psychiatric conditions, active substance misuse, or conditions that make general anaesthesia too high risk are not appropriate candidates. Age alone is rarely an absolute contraindication, and I assess older patients individually based on their functional status and the likely benefit-risk balance. Pregnancy planning within two years of surgery requires specific discussion.
