And it teaches you that the most important thing you can do in a consultation is not to diagnose and prescribe, but to make the parent sitting in front of you understand - because a parent who understands their child's condition is a far more effective partner in that child's care than one who simply follows instructions without knowing why. My Qualifications and Clinical Background
I completed my MBBS followed by my MD in Pediatrics, which gave me the broad clinical foundation across the full range of childhood illness - from newborn assessment and developmental monitoring through to the management of complex chronic conditions. The years of structured training that a paediatric postgraduate programme provides are where you build the clinical thinking that allows you to approach a child's presentation systematically rather than reactively. Fourteen years of practice since then, in Tamil Nadu and serving patients from Andhra Pradesh, has built on that foundation with the kind of experience that only comes from seeing the same conditions across thousands of individual presentations - each one slightly different, each one teaching something.
Why Allergy and Asthma Have Become Central to My Practice
The trajectory of childhood chronic disease over the past two decades has been unmistakeable. Allergic conditions - asthma, allergic rhinitis, eczema, food allergy - are now among the most common reasons children are brought to a paediatrician, and they are also among the conditions most frequently managed inadequately at the primary care level. Children with asthma cycling through antibiotic courses. Children with allergic rhinitis described as having recurrent colds. Children with food allergy restricted from multiple foods without any formal evaluation to confirm which restrictions are genuinely necessary. Addressing these gaps - providing thorough evaluation, accurate diagnosis, and individually tailored management - is the work that has naturally become the specialist focus of my practice.
My Approach to Each Consultation
Allergy and asthma are conditions where the consultation must go beyond the presenting symptom to understand the pattern. A cough that comes on at night and responds to a bronchodilator is telling you something different from a cough that produces coloured sputum and responds to antibiotics. A child who is sensitised to house dust mite and has nasal symptoms year-round that are worst in the morning, in the bedroom, in the school classroom - all high-dust-mite environments - is not having recurrent colds. The history, taken carefully, makes the diagnosis clear long before any test result arrives. I spend time taking that history at every consultation because it is the foundation of everything that follows.
Drug allergy is an area I pay particular attention to in my practice because the consequences of an inaccurate drug allergy label follow a child for a long time. A child who carries an amoxicillin allergy label from a rash during a viral illness may avoid the most appropriate antibiotic for future infections for years or decades, without anyone ever establishing whether the label is accurate. Formal drug allergy assessment addresses this systematically and can safely clarify which labels are genuinely accurate and which can be removed.
