Oligoprogression occurs when one or two sites progress while the majority of the cancer remains controlled on systemic therapy. Rather than stopping an effective CDK4/6 inhibitor, we use SABR to ablate the single progressing site. The patient continues their systemic therapy, which controls the rest of the disease. The AVATAR study supported this strategy, allowing patients to remain on targeted therapy significantly longer before needing escalation to harsher treatment.
