Does hair transplantation cure hair loss permanently?

Hair transplantation produces permanent hair growth in transplanted areas — but it does not stop hair loss in the areas that were not transplanted. Understanding this distinction is essential before any patient decides to pursue surgery, and it is one of the things I spend the most time explaining during pre-transplant consultations.
What hair transplantation does: Restores hair density in the transplanted zone using DHT-resistant donor follicles that produce permanent growth
What it does not do: Stop ongoing androgenetic alopecia in non-transplanted areas. A patient who has a crown transplant but continues losing hair at the frontal hairline will still require medical treatment for the remaining hair
Good candidates: Patients who have stable hair loss, sufficient donor hair, realistic expectations, and who have optimised medical treatment first
My role: I ensure the diagnosis is correct, medical treatment is in place to protect existing hair, and that the patient understands what the surgery can realistically achieve before I refer to a hair transplant surgeon
Modern hair transplant techniques, particularly follicular unit extraction (FUE), involve harvesting individual follicular units from the donor area of the scalp — typically the back and sides, which are genetically resistant to DHT - and implanting them into the thinning or bald recipient area. Because these follicles are DHT-resistant, they continue to produce hair in the recipient site permanently. In this sense, the transplanted hair is permanent.
Ongoing minoxidil and PRP maintenance after hair transplantation is typically recommended to protect existing non-transplanted hair and support the long-term result. I coordinate the medical management around the surgical pathway for patients pursuing this route.

What is the difference between minoxidil and finasteride for hair loss?

Minoxidil and finasteride are the two most established medical treatments for androgenetic alopecia, and they work through entirely different mechanisms. Understanding the difference helps patients know what to expect and why both long-term commitment is essential and discontinuation leads to loss of benefit.
Minoxidil, how it works: Extends the anagen (active growth) phase of the hair cycle and increases the diameter of miniaturised follicles. Available as a topical solution, foam, or low-dose oral tablet
Minoxidil , who can use it: Both men and women. Topical minoxidil is the standard starting point for most hair loss patients. Oral low-dose minoxidil is increasingly used when topical formulations are not well tolerated
Minoxidil, what to expect: Reduction in shedding typically noticed within six to eight weeks. Visible improvement in density over three to six months. Results maintained only with continued use — stopping leads to return of hair loss within months
Finasteride, how it works: Blocks the enzyme (5-alpha reductase) that converts testosterone to dihydrotestosterone (DHT). Lower DHT levels slow follicular miniaturisation and in some cases allow partial regrowth
Finasteride, who can use it: Men with androgenetic alopecia. Not used in premenopausal women who may become pregnant due to the risk of foetal harm
Finasteride, what to expect: Slows progression and may produce modest regrowth. Full benefit seen over 12 to 24 months of consistent use. Like minoxidil, results are lost if treatment stops
For women with androgenetic alopecia associated with PCOS or androgen excess, anti-androgen medications including spironolactone may be appropriate in coordination with a gynaecologist or endocrinologist. I discuss the most appropriate medical hair loss treatment for each patient's diagnosis, age, and profile at every initial consultation.

What causes hair loss in women and how is it different from men?

Women present with hair loss from a significantly wider range of causes than men, and identifying the correct cause before starting any treatment is the most important step. As a Dermatologist with 19+ years of practice in Bengaluru, the most common mistake I see is treatment being started before a diagnosis is confirmed - which wastes time and often misses a treatable underlying cause. 

  • Androgenetic alopecia (female pattern hair loss): Diffuse thinning over the crown with widening of the central parting. Driven by DHT sensitivity of hair follicles, often with a genetic component
  • Telogen effluvium: Diffuse shedding occurring two to four months after a trigger - childbirth, illness, surgery, major weight loss, or severe psychological stress. Post-partum hair loss is the most common presentation I see in women aged 25 to 40
  • Iron deficiency: Even without anaemia, low ferritin is one of the most frequently missed causes of hair loss in premenopausal women and responds well to supplementation Thyroid dysfunction: Both hypothyroidism and hyperthyroidism cause diffuse hair thinning and are entirely treatable once identified
  • PCOS: Elevated androgen levels in PCOS accelerate follicular miniaturisation, producing a pattern similar to androgenetic alopecia, often alongside acne and hirsutism Postmenopausal hair loss: Declining oestrogen shifts the androgenic balance and accelerates pattern thinning in predisposed women - treatable, not inevitable Men primarily present with androgenetic alopecia in predictable patterns. Women require a more thorough hormonal and systemic evaluation before any treatment is recommended. A dermatological hair assessment including trichoscopy and targeted blood tests is the correct starting point.
