expertise
Difficult Gallbladder Surgery

Difficult gallbladder cases arise when severe inflammation, repeated infection, scarring, or anatomical variation makes standard laparoscopic cholecystectomy unsafe to complete without risk of bile duct injury. Dr. Moksha Gowda assesses each case carefully at NiSH Clinic, Jayanagar, Bengaluru, using intraoperative evaluation, appropriate imaging, and the Critical View of Safety principle to determine the safest operative strategy. When standard total removal cannot be safely completed, she employs recognised bailout techniques including subtotal cholecystectomy to achieve the best possible outcome while protecting the bile duct and surrounding structures. Her approach combines technical precision with sound surgical judgement built across 10,000 patient encounters. Written by Dr. Moksha Gowda, MBBS, MS General Surgery, Consultant General and Laparoscopic Surgeon, NiSH Clinic, 48, 7th B Main Road, near Vinayaka Temple, 4th Block, Jayanagar, Bengaluru 560011. Phone: +91 9731174360.

expertise
Subtotal Cholecystectomy

Subtotal cholecystectomy is a deliberate surgical technique in which most of the gallbladder is removed but a small remnant near the bile duct structures is left in place, avoiding the risk of bile duct injury that would arise from forcing a total removal in an unsafe operative field. Dr. Moksha Gowda performs subtotal cholecystectomy at NiSH Clinic, Jayanagar, Bengaluru, as a planned, evidence-based response to intraoperative findings that preclude safe total cholecystectomy. The technique, either reconstituting or fenestrating depending on the anatomy encountered, is selected to minimise the risk of postoperative bile leak while eliminating the risk of bile duct injury. Post-operative management includes structured surveillance imaging and, where a bile leak occurs, coordination of endoscopic or radiological management. Most patients recover fully without requiring further surgery.

expertise
Laparoscopic Cholecystectomy (Gallbladder Removal Surgery)

Laparoscopic cholecystectomy is the surgical removal of the gallbladder using four small keyhole incisions and a camera-guided approach, under general anaesthesia. It is the standard treatment for symptomatic gallstones, acute cholecystitis, biliary colic, and other gallbladder conditions where the gallbladder itself is the source of symptoms. The procedure takes approximately 45 to 60 minutes, most patients go home the same day or the following morning, and the recovery profile is among the fastest of any abdominal operation. Dr. Moksha Gowda, Consultant General and Laparoscopic Surgeon at NiSH Clinic, Jayanagar, Bengaluru, with 14 years of surgical experience and 10,000 patients, performs laparoscopic cholecystectomy for straightforward and complex gallbladder presentations including difficult and inflamed gallbladders, with careful surgical planning and close post-operative follow-up to support the fastest possible safe recovery.

When can I eat normally after gallbladder removal surgery?

The gallbladder stores bile and releases it in response to fat in a meal. After its removal, bile flows continuously from the liver into the small intestine rather than being stored and released in controlled amounts. This means the digestive system has less capacity to process large amounts of fat in a single sitting, particularly in the weeks immediately after surgery. For the first week after laparoscopic cholecystectomy at NiSH Clinic, Jayanagar, I advise patients to follow a light diet of small frequent meals, avoiding fatty or fried foods, full-fat dairy, heavy curries, and large portion sizes. From the second week, most patients can begin to reintroduce a wider range of foods gradually, watching for any symptoms such as loose stools or bloating that suggest a particular food is not yet well tolerated. The majority of patients are able to eat a normal, unrestricted diet within four to six weeks of surgery as the body adjusts to the new pattern of bile flow. A small proportion of patients experience persistent loose stools when eating fatty foods beyond this period and may benefit from longer-term dietary modification. I discuss this at the post-operative review appointment, and if symptoms are persistent, further assessment can help identify whether any additional management is needed.
